Endoscopic ligation as treatment of hemorrhoidal disease

Abstract

ABSTRACT

Introduction: Hemorrhoidal disease is one of the most common reason for consultation in coloproctology. One of the treatments is the band ligation in the outpatient clinic.  We present our experience doing this procedure with the aid of the colonoscopy using the 6 Shooter® Universal Saeed® Multi-Band Ligator.

Aim:  analyze the benefits of internal hemorrhoid endoscopic ligation in patients whom haven’t had improvement with medical and conventional treatment. 

Patients and Methods:  Retrospective study of symptomatic patients with hemorrhoidal disease between grades I-III without response to conventional treatment that underwent endoscopic ligation between February 2015 and February 2020.  All procedures were performed in the endoscopy suite under sedation. 

Results: 146 patients underwent the procedure, 92 were male.  The mean age was 44.0 years old (21-78 SD ± 11.23).  The mean time of the procedure was 12.38 minutes (6-19 SD±2.98).  Twenty four hours after, in a scale from 1-10, patients described a a mean of 4 to describe the pain, 1.5 at 7 days and 0 at day 30.  Three patients suffer from acute urinary retention and 3 patients external thrombosis.  None patient had fever, sepsis or perforation.  

Conclusion:  Endoscopic ligation for hemorrhoidal disease is a reproducible and safe technique that allows treatment of symptomatic hemorrhoids usually in one sesion.
