ANORECTAL SYPHILIS THE GREAT SIMULATOR

Abstract
Background: Syphilis is a chronic systemic infectious-contagious disease causing illness in dozens of millions of people a year. Its polymorphic manifestation makes it a differential diagnosis of benign and malignant anorectal diseases. Objective: Review of the clinical presentation of anorectal syphilis regarding clinical cases. Method: Observational, retrospective, descriptive and literature review study. Results: Most of the patients were men of active sexual age with HIV co-infection. Most frequent manifestation was anal fissure, followed by ulceration. Pseudotumor presentation was infrequent, and stands out for its high diagnostic suspicion given the consequent risk of overtreatment. Conclusion: Syphilis is considered “the great simulator”. Its anorectal presentation as fissure, ulcer and/or pseudotumor is not uncommon. Treatment is simple with widely available antibiotics.
